CHAPTER 4: THE SECOND NAME ON THE CHART

CHAPTER 4: THE SECOND NAME ON THE CHART

The room remained frozen until two hospital security officers stepped inside.

"Ma'am," one of them said calmly to Rachel, "we need you to come with us."

Rachel smiled as though she were the victim.

"There has been a misunderstanding."

The senior nurse folded her arms.

"Then you'll have every opportunity to explain it."

Rachel glanced at Michael one last time.

"I hope you remember who started this."

Michael didn't answer.

He couldn't.

His silence was heavy with guilt.

As security escorted Rachel into the hallway, she turned her head just enough to meet Deborah's eyes.

"You should ask who changed your chart."

Then she disappeared around the corner.

Those words lingered in the room long after she was gone.

Within minutes, two hospital administrators arrived with the head of obstetrics, Dr. Andrew Foster.

Dr. Foster looked genuinely alarmed.

"Mrs. Collins, I'm so sorry."

Deborah clutched Thomas closer.

"I want to know one thing."

"Anything."

"Was Rachel ever supposed to be assigned to me?"

Dr. Foster frowned.

"No."

"What?"

"You were assigned Nurse Emily Watkins for every shift after delivery."

Deborah looked around.

"I've never met anyone named Emily."

The doctor's expression darkened.

"You should have."

A chill swept through the room.

"If Rachel wasn't assigned to me..." Deborah whispered, "...then why was she giving me medication?"

No one had an answer.

Hospital administrators immediately sealed Deborah's room.

Every medication cup, IV bag, syringe wrapper, and chart was collected as evidence.

A pharmacist arrived carrying Deborah's medication records.

She compared the electronic orders with the pills found in the bedside drawer.

After several minutes, she looked up.

"These tablets aren't listed anywhere."

Dr. Foster stared at her.

"What do you mean?"

"They were never prescribed."

Deborah's stomach tightened.

"So what are they?"

"I don't know yet."

"They'll need laboratory testing."

Carol, who had rushed back upstairs after hearing the emergency alarm, covered her mouth.

"Oh my God..."

The pharmacist continued quietly.

"Someone placed medication into this patient's room without a physician's order."

Lily sat beside her mother, refusing to let go of her hand.

A child-life counselor offered crayons and coloring books.

Lily shook her head.

"I'm staying with Mom."

Deborah brushed her daughter's hair behind her ear.

"You were so brave."

Lily finally let herself cry.

"I thought she was going to hurt you."

"You saved us."

"No..."

Lily whispered.

"I just remembered the red button."

Deborah kissed her forehead.

"Sometimes that's exactly how heroes save people."

Meanwhile, Michael sat alone across the room.

He hadn't asked to hold Thomas.

He hadn't tried to explain himself.

He simply stared at the floor.

Finally, Deborah spoke.

"How long?"

He closed his eyes.

"Four months."

Carol inhaled sharply.

Deborah felt almost numb.

"While I was doing fertility treatments?"

"...Yes."

"While I was recovering from my second miscarriage?"

Michael nodded.

Tears rolled down his face.

"I hate myself for it."

Deborah's voice remained eerily calm.

"Did you ever love her?"

"No."

"Then why?"

"I was weak."

"No."

She looked directly at him.

"You were selfish."

He lowered his head.

"I know."

"No."

"You don't."

Hours later, investigators from hospital risk management arrived.

One of them, a former detective named Daniel Perez, carried a thick folder.

"I need to ask everyone a few questions."

He started with Deborah.

Then Lily.

Then Carol.

Finally, Michael.

After nearly an hour, Daniel opened Deborah's electronic medical record.

His eyebrows pulled together.

"That's strange."

"What?"

He turned the monitor toward Dr. Foster.

"Look here."

The room assignment history showed something unusual.

Deborah had originally been scheduled for Room 527.

At 5:42 a.m. on the morning after delivery, someone transferred her to Room 512.

The change had occurred before Rachel's shift began.

"Who authorized this?" Daniel asked.

Dr. Foster frowned.

"It should say."

Instead...

the authorization field contained only initials.

S.M.

"I don't know anyone with those initials," Dr. Foster admitted.

Daniel kept scrolling.

Another entry appeared.

Medication preference updated.

Primary nurse reassigned.

Visitor access modified.

Every change had been approved by the same initials.

S.M.

Carol crossed her arms.

"So Rachel wasn't working alone."

"No," Daniel replied quietly.

"Someone inside the hospital opened every door she needed."

An hour later, security returned.

"We've searched Rachel's locker."

"What did you find?" Daniel asked.

The officer placed several items on the table.

A disposable cellphone.

Photographs of Deborah entering prenatal appointments.

Copies of her prenatal schedule.

Printed ultrasound images.

Even more disturbing...

There were photographs of Lily leaving school with Michael weeks earlier.

Carol gasped.

"She was following them."

Daniel nodded.

"For some time."

Then the officer produced one final item.

A folded piece of paper.

Daniel opened it.

Inside was a handwritten list.

Deborah Collins

Crossed out.

Thomas Collins

Circled.

Below that...

Only three words.

New beginning together.

The room fell completely silent.

Just then, another investigator hurried into the room carrying a laptop.

"You need to see this."

He connected it to the television mounted on the wall.

Hospital security footage filled the screen.

The timestamp showed 5:41 a.m.

The hallway outside Deborah's original room.

Everyone watched as a woman wearing blue scrubs approached the nurses' station.

She typed something into the computer.

Moments later, Deborah's room assignment changed.

The woman turned toward the camera for less than two seconds.

Just long enough to see her face.

Dr. Foster whispered in disbelief.

"No..."

Carol leaned closer.

"Who is she?"

The doctor swallowed hard.

"That's not a nurse."

The room fell silent.

"That's one of our electronic medical records supervisors."

He paused.

"Sarah Mitchell."

Daniel looked down at the initials on the chart.

S.M.

Then back at the frozen image on the screen.

May you like

The conspiracy had just become much bigger than a single obsessed nurse.

Someone with full access to the hospital's medical records had been helping Rachel from the very beginning.
